SCALE Healthcare is committed to maintaining the privacy and security of protected health information (PHI) in accordance with the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and applicable regulations.
Protected Health Information (PHI)
PHI includes any individually identifiable health information that is transmitted or maintained in any form, whether electronic, paper, or oral, and that relates to an individual’s health status, provision of healthcare, or payment for healthcare services.
Our Role
In some instances, SCALE Healthcare may serve as a Business Associate to healthcare providers or organizations (Covered Entities) under HIPAA. In such cases, we enter into Business Associate Agreements (BAAs) and are required to handle PHI in accordance with HIPAA privacy and security standards.
Data Security Measures
We implement appropriate administrative, technical, and physical safeguards to protect PHI against unauthorized access, use, or disclosure. These may include encryption, access controls, secure storage, and staff training.
Use and Disclosure of PHI
SCALE Healthcare does not use or disclose PHI except as permitted or required under applicable BAAs, HIPAA regulations, or with appropriate patient authorization.
No Collection of PHI via Website
This website is not intended to collect PHI. Please do not submit personal health information through online forms or email unless specifically directed through a secure and compliant channel.
Your Rights
Patients have rights under HIPAA regarding their health information, including the right to access, amend, or request restrictions on the use of their PHI. These rights are exercised through the Covered Entity responsible for the PHI.
